A refresh image will take you back to whatever operating system was used to create it. That is, if a refresh image is created in Windows 8.1 and you then perform a refresh, you will still be in Windows 8.1.
If you created a refresh image while running Windows 8 and then upgraded to Windows 8.1, provided you haven't created another refresh image while in Windows 8.1, you could then use that original refresh image to revert back to Windows 8.
