Way back in 1999, I decided to become an organ donor. I don’t know if you have to register anywhere officially. You can check with your state or provincial government website. In Canada, we have a program called “MCP – Medical Care Plan”, so we don’t have to declare bankruptcy with every visit to the doctor.
We do have to renew our cards every few years, and we can register as organ donors by checking the option “Intent To Donate” on the renewal form.
Anyway, I printed up my own organ donor card. I did this in “PhotoFiltre”. Printed it up on photo paper. Trimmed it off. Been carrying it with me ever since.
I can’t remember where I got the template from, but you can adjust it to suit your own needs.
My signature, I wrote it on a piece of paper and then scanned it.
